Online Pre-Registration Requirement

• Copy of the Learner’s Birth Certificate
• Copy of Passport (Learner/s & Parents/Guardian)
• Copy of Visa Page of the Parents/Guardian
  *Visa may not be required for the student so long as at least one paying parents/guardian’s visa page copy is supplied
• Copy of Parent, Family Member or Guardian’s College Diploma or Transcript of Records
  *In order to ensure that the Parent, Family member or Guardian is capable to do the homeschooling
• Copy of Learner’s Form 138 (report cards) from previous completed school year
  *Must be authenticated IF NOT from a previous DepEd Accredited School
• Copy of Transcript of Records (Form 137) (Student’s Latest Completed Academic Year)
  *Must be authenticated IF NOT from a previous DepEd Accredited School

Original Hard Copy Post Registration

* Must be submitted within 15days of starting date of the school year

• Certified True Copy of Learner’s Form 138 (report cards) from previous completed school year
  *Must be authenticated IF NOT from a previous DepEd Accredited School
• Certified True Copy Transcript of Records (Form 137) (Student’s Latest Completed Academic Year)
  *Must be authenticated IF NOT from a previous DepEd Accredited School
